What Your Child will need to attend School This Year

DID YOU KNOW?

- If you have a child in Pre K that we need a Physical with Immunizations and a copy of their Birth Certificate on file.

- If you have a Kindergarten student that we need a physical with immunizations, dental and vision on file.

- If you have a 2^nd grader that we need a dental on file.

- If you have a 6^th grader that we need a physical and dental on file.

- If you have a 9^th grader that we need a physical with current Tetanus booster.

- If your child plays sports that we need a physical yearly on file for them, before open gym's start.

DENTAL SERVICE PROVIDED AT THE SCHOOL

We have a dentist that comes to see all children Pre k- 8^th with a medical card and/or children with no dental insurance. There are forms available in the office at both the Elementary School and Middle School.

SPORT PHYSICALS PROVIDED AT THE SCHOOL

We will again provide sports physicals at the school but this year we are shooting for the week after school gets out. Look for information with your registration packet.
